CLIENT INTAKE CONSeNT FORM - reiki and sound bath
Reiki is a Holistic Practice, treating the whole person including body, emotions, mind and spirit, creating many beneficial effects that include relaxation and feelings of peace, security and wellbeing.
Important: I understand that my session with Angelina Papas as a Holistic Integrative Creative Therapist & Energy Healing Practitioner is not intended as a substitute for medical or psychological diagnosis or treatment. I understand that any information shared within the session being confidential, is derived from a Holistic approach combined with professional guidance.
I am affirming that prior to becoming a client, I was given sufficient information to understanding the nature of Reiki, is practiced in a complimentary capacity to relieve stress, anxiety, clear & bring to balance the emotional, mental, physical & spiritual aspects of the body.
I acknowledge as advised that post sessions it is recommended to take some quiet time to allow whatever has transpired energetically and or emotionally to shift and release. (this maybe in the form of dreams, journaling, insight, or physical bowel movement).
Do not stop any treatment which you are currently receiving without the prior consent of your medical practitioner.
*Cancellation Policy, a minimum of 48 hours’ notice, or a 25% session fee will be charged.
